نبذة مختصرة : Background: The problem of inequalities toward access to health care is major in French Guiana, especially the retention in care of precarious population with chronic pathologies. The Permanent Access to Health Care (PASS) in Cayenne, offers medical and social follow-up and accompanies to the common law healthcare system. The main objective is to evaluate the continuity of care of the PASS patients with open rights. Method: It is observational research conducted at Cayenne hospital with structured interview with all the hypertensive and/or diabetic patients followed by the PASS and reoriented to the general practitioner between January and November 2022. The proportion of patients integrating the common law system is defined by resort to consultations with a general practitioner, use of the town pharmacy, the performance of additional examinations if requested. Results: 133 patients were included, all of whom were foreigners, 1/4 newcomers, 1/2 French-speakers. 7 out of 10 patients had “full integration” into the common law system one month after leaving PASS. Despite an overall “good compliance”, 31.2% did not have a “good understanding” of chronic disease monitoring. The pathology was not stabilized at the end of the PASS for 42.4% of the patients. Conclusions: The Cayenne PASS takes care of particularly precarious patients compared to the European French territory PASS. Its role as a bridge is well fulfilled thanks to the systems already in place (health mediators) but the PASS needs a formalized follow-up procedure (identification of predictive profiles of failure, the creation of workshops).
