Reduction of health status disparities in primary care: systematic review ; Réduction des inégalités sociales de santé en soins primaires : revue de la littérature

Introduction: Social Inequalities in Health (SIH) reflect the differences in morbidity and mortality between social groups and across the population on a social gradient. The objective of the study is to list the effective and reproducible interventions feasible in primary care for the reduction of the social gradient of health. Methods: Key words in relation to primary care and the social status of patients have allowed the MEDLINE database to be interrogated and go through the grey literature. Results: 9 Articles on 544 were retained. The multi-professional interventions, health education and coaching methods, prevention and therapeutic education and those aimed at increased communication and inter-professional collaboration are effective. Discussion: To optimize anti-SIH interventions, it would be important to develop the concept of "Proportionate universalism" and specify their method of assessment. Conclusion: The General Practitioner whose role is central in the reduction of SIH, must be better trained and have the means to act. ; Introduction : les Inégalités Sociales de Santé (ISS) reflètent les écarts de morbidité et de mortalité entre groupes sociaux et traversent la population selon un gradient social. L’objectif de l’étude est de lister les interventions efficaces et reproductibles réalisables en soins primaires permettant la réduction du gradient social de santé. Méthodes : les mots clefs en rapport avec les soins primaires et le statut social des patients ont permis d’interroger la base de données MEDLINE et de parcourir la littérature grise. Résultats : 9 articles sur 544 ont été retenus. Les interventions pluriprofessionnelles, ainsi que celles s'appuyant sur des démarches d'éducation à la santé et de coaching, de prévention et d'éducation thérapeutique et visant l’amélioration de la communication et de la collaboration inter professionnelle sont efficaces.
