Effect of integrated traditional Chinese and western medicine nursing interventions based on enhance recovery after surgery in patients undergoing arthroscopy (基于ERAS理念的膝关节镜手术围手术期中西医结合护理效果观察)

Objective To investigate the effect of integrated traditional Chinese and western medicine nursing interventions based on the enhance recovery after surgery in patients undergoing arthroscopy. Methods A total of 100 patients undergoing arthroscopy for knee osteoarthritis were randomly divided into the control group and observation group, with 50 cases in each group. Patients of the control group received routine nursing after arthroscopy, and those of the observation group were given integrated traditional Chinese and western medicine nursing interventions based on enhance recovery after surgery. The first exhaust time and defecation time after surgery, Visual Analogue Scale (VAS) on pain, the Hospital for Special Surgery Knee Score (HSS) and patients’ satisfaction with nursing interventions were compared between two groups. Results Patients of the observation group had a shorter first exhaust time and defecation time compared with those of the control group (P＜0. 01). Patients of the observation group had lower VAS score at 24 hours and 48 hours after surgery compared with those of the control group (P＜0. 01). Patients of the observation group had higher HSS score at 24 hours and 48 hours after surgery compared with those of the control group (P＜0. 01). A higher degree of patient satisfaction was associated with integrative nursing based on enhance recovery after surgery(P＜0. 05). Conclusion The integrative nursing based on enhance recovery after surgery is effective to improve the recovery of gastrointestinal functional, relieve the postoperative pain and anxiety, and improve the knee joint function after arthroscopy. (目的 探讨基于加速康复外科(ERAS)理念的膝关节镜手术围手术期的中西医结合护理效果研究。方法 选取2024年1月—9月医院收治的膝关节骨性关节炎行膝关节镜手术患者100例为研究对象, 随机分为对照组和观察组, 各50例。对照组按照骨伤科围手术期护理常规进行护理; 观察组在对照组基础上基于ERAS理念实施中西医结合护理干预。记录两组患者术后首次排气和排便时间, 采用视觉模拟评分(VAS)评估术后疼痛, 采用美国特种外科医院(HSS)评分评估膝关节功能, 采用自制量表调查满意度。结果 与对照组比较, 观察组患者术后首次排气和排便时间缩短, 差异有统计学意义(P＜0. 01)。观察组术后24 h和术后48 h的VAS评分低于对照组, 差异有统计学意义(P＜0. 01)。观察组出院时和出院后14 d的HSS评分均高于对照组, 差异有统计学意义(P＜0. 01)。观察组护理满意度高于对照组, 差异有统计学意义(P＜0. 05)。结论 在常规围手术期护理基础上, 基于ERAS理念的中西医结合护理干预能够缩短术后排气时间和排便用时, 减轻术后疼痛、焦虑程度, 促进患者膝关节功能的恢复。)
