نبذة مختصرة : Objective To investigate the quality and safety of Traditional Chinese Medicine(TCM) nursing in 35 secondary and above public medical institutions in Zunyi City, grasp the current situation of TCM nursing in medical institutions, analyze the causes of problems in TCM nursing work, and achieve the purpose of improving the quality of TCM nursing work. Methods Using the evaluation criteria of grade hospitals as the investigation tool, questionnaire survey and structured interview were used to collect data. Results A total of 35 valid questionnaires were collected in this survey, and the effective questionnaire recovery rate was 100. 00%. The survey covers 35 secondary and above TCM hospitals, integrated traditional Chinese and Western medicine hospitals and general hospitals in Zunyi City, including 16 tertiary and above hospitals (45. 71%), 19 secondary hospitals (54. 29%); Including 19 general hospitals (54. 29%), 15 TCM and integrated traditional Chinese and Western medicine hospitals (42. 86%), one maternal and child health care hospital (2. 85%). In this survey, the actual number of open beds in TCM and integrated traditional Chinese and Western medicine hospitals was 7096, with a total of 3122 nurses, the ratio of bed to nurse in TCM hospitals was 1: 0. 45; The actual number of open beds in general hospitals was 12, 937, with a total of 8, 861 nurses, and the ratio of bed to nurse in comprehensive hospitals was 1: 0. 68. There were 912 nurses graduated from traditional Chinese medicine colleges, 394 (12. 62%) from traditional Chinese medicine hospitals and 518 (5. 85%) from traditional Chinese medicine colleges in general hospitals. The survey results showed that 202 nursing technology units were carried out in TCM and integrated Chinese and Western medicine hospitals, with an average of 14 items; A total of 52 TCM nursing technical units were carried out in Western medicine hospitals, with an average of 3. A total of 403 TCM nursing programs were carried out in TCM hospitals, with an average of 28 programs. A total of 99 TCM nursing programs were carried out in Western medicine hospitals, with an average of 4, among which 7 general hospitals did not carry out TCM nursing programs; A total of 369 TCM nursing techniques were carried out in TCM hospitals, with an average of 20; A total of 146 TCM nursing techniques were carried out in Western medicine hospitals, with an average of 7. This survey randomly selected 15 western medicine or traditional Chinese medicine, integrated Chinese and Western medicine hospitals, respectively, to assess the quality of traditional Chinese medicine characteristics, ward management and safety quality, nursing documents, disinfection and isolation quality and the quality of traditional Chinese medicine nursing work in Western hospitals(with a full score of 100 points), the average assessment score of traditional Chinese medicine and integrated Chinese and Western medicine hospitals is 83. 68 points. The average score of Western hospital assessment is 51. 00points. Conclusion There are some problems in the city's medical institutions, such as insufficient ratio of TCM nursing human resources, imperfect TCM nursing program, incomplete development of TCM appropriate technology, quality management of TCM nursing needs to be improved, and insufficient attention and professionalism of TCM nursing work in general hospitals. Compared with the data in 2020, the city's medical institutions are paying more attention to TCM nursing work, but it is still necessary to strengthen the ratio of TCM personnel allocation, construct and optimize the use of TCM nursing programs, fully carry out TCM appropriate technologies, strengthen the supervision and supervision of nursing quality management, strengthen exchanges between Chinese and western hospitals, and improve the construction of TCM services in general hospitals, in order to improve the quality of TCM nursing work in hospitals. (目的 对遵义市35家二级以上公立医疗机构开展中医护理质量安全工作调研, 掌握医疗机构中医护理工作现状, 分析中医护理工作中问题产生的原因, 达到提升中医护理工作质量的目的。方法 以等级医院评审标准为调查工具, 采用问卷调查法、结构化访谈法进行数据收集。结果 本次调查回收有效问卷35份, 有效问卷回收率为100. 00%。本次调查涵盖遵义市35家二级以上中医医院、中西医结合医院、综合医院, 其中三级以上医院16(45. 71%)家, 二级医院19(54. 29%)家; 包含综合医院19(54. 29%)家, 中医、中西医结合医院15(42. 86%)家, 妇幼保健院1(2. 85%)家。本次调查中医、中西医结合医院实际开放床位数7096, 护士总数3122名, 中医类医院床护比1: 0. 45, 综合医院实际开放床位数12937, 护士总数8861名, 综合类医院床护比1: 0. 68; 中医院校毕业护士总数912名, 中医院中医院校毕业护士394(12. 62%)名, 综合医院中医院校毕业护士518(5. 85%)名。调查结果显示, 中医、中西医结合医院中共开展中医护理操作技术护理单元202(79. 53%)项, 平均为14项; 西医医院开展中医护理操作技术护理单元共52(20. 47%)项, 平均为3项。中医、中西医结合医院共开展中医护理方案403(80. 28%)项, 平均28项; 西医医院中医护理方案共开展99(19. 72%)项, 平均4项; 其中7家综合医院未开展中医护理方案。中医、中西医结合医院共开展中医护理技术369(71. 65%)项, 平均20项; 西医医院中医护理技术共开展146(28. 35%)项, 平均7项。本次调研随机抽取15家西医或中医、中西医结合医院, 分别对中医特色质量、病区管理与安全质量、护理文书、消毒隔离质量以及西医医院中医护理工作质量进行考核评定(满分100分), 中医、中西医结合医院平均考核得分83. 68分, 西医医院考核平均分为51. 00分。结论 全市医疗机构均存在中医护理人力资源配比不足, 中医护理方案不健全, 中医适宜技术开展不全面, 中医护理质量管理工作有待提升, 综合医院中医护理工作重视度不足、专业性不强等问题。相较2020年数据, 全市医疗机构对中医护理工作的重视度在不断提升, 但还需加强中医人员配置比, 构建并优化运用中医护理方案, 全面开展中医适宜技术, 加强护理质量管理监管与督导, 加强中西医医院之间的交流, 提高综合医院中医药服务建设, 内培外引以提高各医院中医护理工作质量水平。)
