نبذة مختصرة : BACKGROUND: Intermediate Respiratory Care Units (UCRI) have experienced growth. We analyze two UCRI from the same health group located in hospitals with different epidemiological situations and level of complexity. HYPOTHESES: (1) UCRI are effective and efficient. (2) UCRI during the pandemic have been key. (3) The costs of a UCRI before and during the pandemic, regardless of complexity, are lower than those of an ICU. OBJECTIVES: To carry out an analysis of the costs of a highly complex UCRI and a low complexity UCRI to know the annual expense associated with the cost avoided with respect to ICU stays, in a non-pandemic and pandemic scenario. Secondary Objectives: To describe the epidemiological, clinical and prognostic factors associated with mortality of hospitalized patients in a UCRI. MAIN RESULTS: In reference to the UCRI of a tertiary hospital in a non-pandemic situation: The UCRI avoids a cost of € 500,000 / year. The cost per day of stay at the UCRI was € 380 per patient. Costs are associated with average weight and mortality. Respiratory rate, leukopenia, anemia, hyperkalemia and acidosis are associated with mortality. In reference to the UCRI of a secondary hospital in a situation of pandemic due to COVID-19: The total cost of the UCRI amounted to € 66,233. The cost per day of stay at UCRI was € 164 per patient. The total cost avoided was €214,865. Relationship with respect to mortality: fever ≥ to 39 ◦C, protocolized pharmacological treatment and IoT. CONCLUSIONS: The UCRI analysed have proven to be efficient in terms of «cost avoided» and effective with respect to mortality ; ANTECEDENTS: Els orígens de les Unitats de Cures Respiratòries Intermèdies (UCRI) van lligades als inicis de la Ventilació Mecànica no Invasiva (VMNI).
