MODEL IZVAJANJA PROCESA ZDRAVSTVENE NEGE S POMOČJO EKSPERTNEGA SISTEMA ; MODEL OF EXECUTING NURSING PROCESS WITH THE HELP OF EXPERT SYSTEM

Dokumentacija procesov zdravstvene nege predstavlja glavni vir informacij za odločitve, katere zdravstveni tim sprejema v času zdravstvene oskrbe pacienta. Uporabnost dokumentacije zdravstvene nege je odvisna od strukture in predstavitve informacij zdravstvenemu timu. Prav tako, pa oblika strukture dokumentacije zdravstvene nege narekuje zmožnost uporabe ne zgolj za namene predstavitve, temveč tudi za avtomatizacijo (sistemi za pomoč pri odločanju), integracijo in ponovno uporabo podatkov zdravstvene nege med različnimi zdravstvenimi aplikacijami. Dokumentiranje procesov zdravstvene nege predstavlja ključni problem zdravstvene nege, kot temeljne sestavine celovitega sistema zdravstvenega varstva. Predstavlja zelo pomemben dejavnik pri ustreznem obvladovanju velikega števila zahtevnih in vedno spreminjajočih informacij pri načrtovanju, izvajanju in vrednotenju zdravstvene nege. Uporaba sodobne informacijske tehnologije ponuja možnost zagotavljanja podpore ponovni uporabi znanja v procesu zdravstvene nege preko priporočilnega ekspertnega sistema ter prilagajanju strukture dokumentacije zdravstvene nege skladno z razvojem in novimi spoznanji. Z uporabo sodobne informacijske tehnologije in pristopov, je potrebno omogočiti enostaven in uporabniku prijazen pristop k modeliranju dokumentacije zdravstvene nege ter nudenju podpore odločanja v vseh fazah izvajanja procesa zdravstvene nege. V okviru doktorske disertacije je bil glavni namen opredeliti inovativen model za razvoj zdravstvene dokumentacije ter zasnovo priporočilnega ekspertnega sistema zdravstvene nege. Disertacija v uvodu predstavlja pregled ključnih raziskovalnih področji razvoja elektronskega zdravstvenega zapisa, zdravstvene nege in predstavitve znanja v informatiki. Predstavljamo aktualne aktivnosti v smeri standardiziranega elektronskega zdravstvenega zapisa, njegove elektronske izmenjave in načinov implementacije. Nadaljujemo z opredelitvijo zdravstvene nege, njenega procesa in dokumentiranja v okviru elektronskega zdravstvenega zapisa. Temu sledi pregled priporočilnih ekspertnih sistemov in načinov delovanja ter smiselnost uporabe le teh na zdravstvenem področju, točneje na področju zdravstvene nege. Sledi predlagani ontološki model zdravstvene nege, kjer najprej predstavimo osnovne elemente graditve ontologije v multi-relacijskem lastnostnem grafu, za tem pa predstavimo štiri-nivojski meta model. Na osnovi ontologije zdravstven nege predstavljamo priporočilni sistem, kot podporo pri reševanja problemov, ki nastopajo v posamezni fazi negovalnega procesa. Poseben poudarek dajemo procesnemu modelu izdelave ontologije in detajlnem prikazu ontološkega modela zdravstvene nege na praktičnem primeru. Disertacijo zaključujejo rezultati implementacije ontološkega modela zdravstvene nege v praksi in kritična analiza rezultatov. Ključni prispevek disertacije predstavlja štiri nivojski meta model ontologije ZN, na osnovi katerega je moč preko predlaganega procesa razvoja ontologije, razviti model dokumentacije ZN brez poglobljenega tehničnega znanja in znanja načrtovanja ontologij. Na osnovi ontologije smo podali tudi model priporočilnega ekspertnega sistema, ki na podlagi egocentričnega pristopa sprehajanja skozi domenski graf, omogoča reševanje problemov (priporočilne točke) in podajanje želenih informacij znotraj posamezne faze procesa ZN ter je s tem v pomoč pri odločitvah, ki jih mora medicinska sestra sprejemati pri svojem delu, obenem pa zmanjšuje možnost napak. Ontološki model in priporočilni sistem ZN, sta bila prototipno implementirana, preizkušena v praksi in kritično ovrednotena. Raziskovalno delo odpira priložnosti za nadaljnje raziskovanje predvsem v smeri podrobnejše določitve splošnega modela zdravstvene nege v širšem prostoru zdravstvenega varstva, razširitev algoritmov priporočanja in možnost predstavitve modela v drugih ontoloških jezikih OWL/RDF/RDFS ter zadostitev standardom interoperabilnosti openEHR in HL7. ; Healthcare process documentation represents the main source of information for decisions, that medical team make in the time of patient care. Nursing care documentation usability depends on its structure and type of presentation to healthcare team. Structure of the nursing documentation also has major impact into ability to use documentation not only for presentation purposes, but also for automation (intelligent systems and decision support), integration and reuse of data between different healthcare applications. Documenting processes of nursing care is a key problem of nursing as a fundamental component of a comprehensive health care system. It represents a very important factor in the proper management of a large number of complex and ever-changing information in steps of planning, implementation and evaluation of nursing care. The use of modern information technology offers the possibility to provide support for the re-use of the nursing knowledge in nursing process and use of the expert system. By using modern information technology and approaches, it is necessary to enable an easy and user-friendly approach to modelling the documentation of nursing care and providing decision support in all stages of the nursing process. In dissertation, the main purpose is to identify innovative model for the development of medical documentation and the design of recommendation expert system of nursing care. In the introduction we present an overview of the key research areas of electronic health record development, nursing care and the presentation of knowledge in the filed of informatics. We present the current activities in the process of standardized electronic medical records, electronic data exchange and its methods of implementation. Next the definition of nursing, its processes and documentation in the context of electronic medical records are presented. This is followed by an overview of recommendation systems and possibility of using recommendation methods in the medical field, specifically in the field of nursing. Next we proposed nursing ontological model by stating the basic building elements in multi-relational property graph, through four-level meta-model. Based on the nursing ontology we presented recommendation experts system for adding support in solving problems that occur in each phase of the nursing process. Particular emphasis is placed on process of ontology development. The last section explains results of the implementation of the proposed nursing ontological model and recommendation expert system in practice using SWOT analysis. A key result of the thesis presents four level meta-model for building nursing ontology and consequently developing documentation model of nursing care without in-depth technical knowledge and skills. Based on ontology, we modelled a recommendation expert system based on egocentric approach that helps nurse solving problems in each step of the nursing process and thereby assist in the decision making, while reducing the possibility of errors. Ontological model and recommendation expert system was implemented and tested in practice and also critically evaluated . The research work opens up many opportunities for further research especially in the direction of establishing more comprehensive ontological nursing model, better recommendation algorithms the possibility of presenting nursing ontology model in different languages OWL/RDF/RDFS and meet the interoperability standards open EHR and HL7 .
