Akut brusellozlu bir çocukta geçici hemofagositoz

Although brucellosis is a rarely seen zoonotic disease in developed countries, it is still a health problem in Turkey. Pancytopenia is an infrequent complication of brucellosis. A fifteen-year-old girl was referred to the hospital with the complaints of prolonged fever, weakness and arthralgia. Physical examination and laboratory tests showed hepatosplenomagaly, subconjuctival hemorrhagia and hemophagocytosis associated with reactive histiocytosis in bone marrow. Blood culture was positive for Brucella melitensis. She was treated with the combination of doxycycline and rifampin and improved in a short period. Pediatricians should consider the diagnosis of brucellosis in the differential diagnosis of any case with prolonged fever, hepatosplenomegaly and hemophagocytosis. ; Bruselloz, gelişmiş ülkelerde seyrek görülmesine karşın, Türkiye'de halen halk sağlığı sorunu olmaya devam eden zoonotik bir infeksiyon hastalığıdır. Pansitopeni brusellozun nadir görülen bir komplikasyonudur. Onbeş yaşında kız hasta; uzamış ateş, sol gözde kanama ve halsizlik nedeniyle başvurdu. Fizik muayene ve laboratuvar incelemeleri sonucu hastada subkonjunktival hemoraji, hepatosplenomegali, pansitopeni, kemik iliği aspirasyonunda hemofagositoz ve histiyositoz saptandı. İnfeksiyon ilişkili hemofagositik sendrom düşünülen hastadan alınan hemokültürde Brucella melitensis üredi. Doksisiklin ve rifampisin tedavisi ile hasta hızla iyileşti, hematolojik bulguları normale döndü. Bu olgu sunumu ile uzamış ateş, hepatosplenomegali ve hemofagositoz ile seyreden bruselloz olgularının görülebileceği; brusellozlu hastalarda hemofagositozun geçici olduğu ve uygun tedavi ile kısa sürede düzeleceği vurgulanmıştır.
