      Purpose: this study aims to evaluate the screening performance of the Mini-Cog test among outpatients seeking consulting for memory-related concerns.Method: we conducted a retrospective analysis of a 7-years longitudinal monocentric cohort from the Nice Memory clinic was performed. Patients were categorized in three groups: a) controls, b) minor Neurocognitive disorder (NCD), c) major NCD, following a standardized diagnostic process. Mini-Cog scores were calculated using the 3-words recall from the Mini-mental state examination (MMSE) and results from the Clock drawing test (CDT).Results: the total sample included 987 patients, with 636 (64,4%) being women, and a mean age of 82,3 ± 8,3 years. We identified a positive correlation between the Mini-Cog and the MMSE scores (Pearson Coefficient r = 0,69 (p < 0,001)). Using the original cutoff of ⩽ 2, the Mini-Cog exhibit a sensitivity of 0,83 (CI95%[0,81-0,86]), a specificity of 0,69 (CI95%[0,58-0,78]), a positive likelihood ratio (LR+) of 2,68 (CI95%[1,96-3,67]), and an area under the curve (AUC) of 0,86 for detecting NCD, including minor NCD. Employing a cutoff of ⩽ 3 increase sensitivity by 10% (Se = 0,93 CI95%[0,91; 0,94]).Conclusion: the Mini-Cog is a brief, reproductible, and effective screening tool for neurocognitive disorders suitable for primary care practice. Its primary advantage lies in the early identification of potential NCD patients, including those with minor NCD, enabling the initiation of comprehensive care at the earliest signs of neurocognitive disorders.
