      The sustainability of supply chains in rural areas presents greater challenges due to the means and modes of transport like long distances, the service networks and geographical conditions. In Colombia, the situation is particularly reflected in remote areas such as the department of Chocó, where the geographical dispersion is 5.4 times higher than the rest of the country's department. The present investigation proposes a mathematical model for the location of fixed and mobile Health centers as a case study the Municipality of Bahía Solano, Chocó and compares its results with the brigades or mobile Health Days that are carried out in the Municipality. The brigades destined for the provision of health services in areas of difficult access in Colombia. The methodology developed in the research is mixed. The research combines qualitative and quantitative approaches. Initially, at investigations, development the characterization of the health technology supply network of the Bahía Solano, Municipality of the Department of Chocó. This is presented through primary and secondary sources of information. Subsequently, a deterministic mathematical model of linear programming is proposed for the location of fixed and mobile Health centers. The mathematical model is presented as a tool for locating health centers and as a comparison of the current system of health conferences or brigades in rural areas. What contributes to not only to the economic benefits of obtaining lower costs within the supply chain of health technologies. Also, the research contributes to social impact for people living in these areas of greater vulnerability.
