نبذة مختصرة : Background: the commonest origin of acute urinary retention is benign prostatic hyperplasia resulting in bladder outflow obstruction. Purpose: to assess long-term results of prostate artery embolization (PAE) for acute urinary retention related to benign hyperplasia. Materials and Methods: we performed a retrospective study of all consecutive patients who underwent PAE for acute urinary retention (AUR) from August 2011 to December 2021 in a single institution (n=88; mean age = 72 years ± 12). Patients underwent a first attempt of catheter ablation two weeks after PAE. Clinical success was defined as the absence of AUR recurrence. Results: successful catheter removal in the month following PAE was observed in 82 % (72/88) of the patients. Clinical success persisted for 66% (58/88 patients) at long-term follow-up (19.5 months ± 16.5 [range : 2 -74]). Recurrence occurred in 16% (14/88) patients with initial catheter removal and occurred at 16.2 months ± 12.2 [range: 1.5 – 43] post-PAE. Overall, 21/88 (24%) patients of the cohort underwent prostatic surgery, with surgery-free survival of 10.4 months ± 12.2 [range : 1.2 – 42.4]. Kaplan-Meier analysis provided a three-year catheter free survival probability of 60%. Conclusion: prostate artery embolization is a valuable technique for acute urinary retention related to benign prostatic hyperplasia in case of failure of trial without catheter.
