Medication reviews in hospitals: implementation in psychiatric day hospitals ; Bilans de médication en milieu hospitalier : mise en place en hôpitaux de jour psychiatriques

Medication Reviews (MR), carried out for several years in community pharmacies, are still barely developed at hospital. The purpose of this study was to define an organization to set up MR within two psychiatric day hospitals of our health institution. Following a multi-professional meeting during which the project was presented, a team composed of a nurse and a pharmacy resident was created in order to lead the MR. During the 1st MR interview, the best possible medication history is established and approved by the patient during a 2nd interview 3 weeks later. A summary letter containing pharmaceutical interventions is sent to prescribers. It also contains informations regarding the patient pathologies and treatments. A final interview is held after 3 months to perform a follow-up. A first evaluation at 6 months shows patients satisfaction concerning these interviews and allowed the validation of the organization tested for the implementation of MR in psychiatric day hospitals. It would be interesting to collect the feedback from primary care physicians regarding the pharmaceutical interventions released. The collaboration between the community and the hospital could be reinforced by offering dispensing community pharmacists the opportunity to carry out follow-up interviews at 3 months. MR in psychiatric day hospitals provide to patients a new type of pharmaceutical care aiming to the reduction of drug-related iatrogenic events. ; Les Bilans De Médication (BDM), réalisés depuis quelques années en officines, restent encore très peu développés à l’hôpital. Dans cette étude, nous avons cherché une organisation afin de réaliser des BDM pour des patients pris en charge dans deux Hôpitaux De Jour (HDJ) psychiatriques de notre établissement.
