Objective: To determine non-ionic linear structure gadolinium- based contrast agents (GBCA) accumulation in the brain structures in patients with normal renal function. Research tasks: To assess signal intensity (SI) difference in axial MRI T1- weighted images in the dentate nucleus, globus pallidus, thalamus and caudate nucleus before GBCA administration, after first, second and >6 gadodiamide doses and to asses correlation with the number of GBCA‘s administrations, total amount, time between first and last MRI or mean interval between MRIs. Methods: In this retrospective studies we analysed 16 children with multiple sclerosis. We assessed SI of the dentate nucleus, globus pallidus, thalamus and caudate nucleus in the T1- weighted images after first, second, third and 7-8 MRIs and evaluated the correlation of the changes with the number of gadodiamide administrations, total amount, time between first and last MRI or mean interval between MRIs. Statistically significant differences were estimated if p < 0.05. Results: After GBCA administrations mean SI in the dentate nucleus (p<0.001), globus pallidus (p=0.001) and thalamus (p=0.039) were statistically significantly different but not in the caudate nucleus (p= 0.144). After > 6 gadodiamide doses SI was significantly different in all analysed structures (p= 0.005). SI is not significantly correlated with the number of GBCA‘s administrations, total amount, time between first and last MRI or mean interval between MRIs (r= -0.320- 0.451, p > 0.05). Conclusions: Administration of non-ionic linear GBCA makes difference in SI of dentate nucleus, globus pallidus and thalamus. SI after > 6 gadodiamide doses was different in all analysed brain structures. Hyperintensities were not dependent on GBCA‘s dose or the time of administrations. Recommendations: European Medicines Agency (EMA) recommends using the smallest effective doses of macrocyclic GBCA in MRI.
