      Introduction: In a chronic condition perspective, living with HIV and AIDS infection changed the illness experience and required health practices redirection in order to qualify the care and detached the need to search strategies that corroborate for the epidemic confrontation as the clinic risk stratification. Objective: build an instrument for people living with HIV and AIDS clinical risk stratification. Method: Methodological study based on the Instrument Construction Model, covering the theoretical phase and of construction. Developed through the conceptual analysis model proposed by Walker and Avant, the content validation by expert judges and the items semantic validation by the nurses of the HIV and AIDS reference service. The Delphi technique was applied as well as the Content Validity Index and Cronbach Alpha to verify the instrument validity and confidence. This study was approved by the Ethics Committee in the Health Science Center, at Universidade Federal da Paraíba, under number 3.357.731, CAAE: 12939319.8.0000.5188. Results: The instrument produced, called ERCHAids, was composed by 18 items distributed in five different areas. A score was developed to calculate the risk percentage of the individuals for undesirable clinical outcomes, divided into four strata: low, medium, high and very high risk. In the content validation the instrument had got an excellent concordance among the specialists with content validity index of 0,954 and 0,916 for items pertinence and appropriateness, respectively, and excellent Cronbach Alpha of 0,850. The semantic validation carried out by the nurses’ of the service showed agreement level over 0,900 for items pertinence and appropriateness, showing that the instrument is valid for the proposed objective. Conclusion: This instrument could guide the care planning, qualify the clinical management according to the need of each person living with HIV and AIDS, and contribute to the management and conscious use of the health resources. ; Coordenação de Aperfeiçoamento de ...
