This paper presents some psychological considerations to consider when surgically treating obesity with bariatric surgery; it incorporates a comprehension of the body according to the three orders that Jacques Lacan formalized: real, symbolic, and imaginary. It analyzes the procedure in the pre- and post-operative stages and recognizes the importance of adding the notion "symbolic body" and examining the treatment by integrating the aforementioned orders. Such a proposal can be defined as a "supplementary complement" regarding the current therapeutic alternatives; it complements the approaches available but in a supplementary way by allowing the various forms of comprehending the phenomenon and encouraging the use of new therapeutic possibilities. The paper also presents some current problems due to not including the psychoanalytic notion "symbolic body" in the medical-surgical treatment. It finally presents some ideas to advance the comprehension of "morbid obesity" by incorporating the three body dimensions previously mentioned. ; Este trabajo expone consideraciones psicológicas a ser tenidas en cuenta en el tratamiento quirúrgico de la obesidad mediante cirugía bariátrica, incorporando una comprensión del cuerpo en los tres registros formalizados por Jacques Lacan: “Real-Simbólico-Imaginario”; analiza la intervención corporal en las fases pre y post operatoria, reconociendo la importancia de anexar la noción de “cuerpo simbólico” y de examinar el tratamiento al integrar los registros mencionados. Lo propuesto se define como un “complemento-suplementario” ante las alternativas terapéuticas existentes hoy, complementando los abordajes disponibles, pero de modo suplementario sin clausurar diversas maneras de comprensión del fenómeno y estimulando la habilitación de nuevas vías terapéuticas.
