A prospective hospital-based study to examine the link between microvascular complications and different glycemic indices in type 2 diabetes mellitus patients

      Background: Microvascular problems have a major impact on morbidity, mortality, and patients' quality of life in type 2 diabetes. There are a few clinical trials in this area, but most of them lack sufficient power and are limited to a single issue. The purpose of this study was to see if there was a link between microvascular problems and diabetes duration and glycemic indices (Fpg, 2hpg, and HbA1c) in type 2 diabetes mellitus patients. Materials & Methods: This is a hospital-based prospective study of 100 patients with type 2 diabetes mellitus at the upgraded department of medicine at Darbhanga Medical College Hospital, Darbhanga, Bihar. Each participant got a thorough clinical examination and detailed history. Diabetes was diagnosed using the new American Diabetes Association (ADA) criteria. Relevant investigations were performed on the selected individuals to determine the presence of micro complications such as diabetes retinopathy, diabetic nephropathy, and diabetic neuropathy. Results: Our study found that the fasting blood glucose level, 2 hr. postprandial glucose level, and HbA1c were not significant correlates, but that having diabetes for more than 10 years had a higher mean value than having diabetes for less than 5 years. The spearmen correlation study demonstrates the relationship between several risk variables and diabetic neuropathy, diabetic retinopathy, and diabetic nephropathy. Fasting blood glucose level, 2 hr postprandial glucose level, and HbA1c were all significant correlates (P<0.0001, p<0.0001, and p<0.0001, respectively). Conclusion: We concluded that using spearman correlation for microvascular problems, we found a positive link for patient age, diabetes duration, fasting blood sugar, 2 hr. postprandial glucose, and HbA1C.
