      Metacarpophalangeal (MCP) impairment is often associated with inflammatory or crystalarthropathies, but its frequency and the factors associated with osteoarthritis (OA) of thesejoints are poorly known. Our objective is to describe the frequency and the characteristics associated with MCP OAamong patients with symptomatic hand OA in DIGICOD cohort.MCP OA was defined by at least 2 MCP with a Kellgren and Lawrence score=2. Its prevalence and distribution were described. We analyzed epidemiological and hand OA characteristics associated with MCP OA by logistic regression in univariate and multivariate models.MCP OA is frequent (32,5%) and predominates in dominant hand (40,6% vs 34,7%, p=0,012). However, it is not frequently symptomatic and structurally not severe. It is independently associated with a higher age, manual occupations and to a higher radiographic severity of hand OA at proximal interphalangeal ant scapho-trapezial joints. No association were found between MCP OA and metabolic factors, nor with clinical severity. In this cohort of patients with severe symptomatic hand OA, radiographic MCP OA was frequent, was associated with structure but not with symptom severity and related to mechanical rather than systemic factors. The presence of exclusive, predominant or erosive impairment of MCP should make us reconsider the diagnosis of hand OA and look for an inflammatory or a metabolic rheumatism. ; L’atteinte des métacarpophalangiennes (MCP) est fréquente au cours des rhumatismes inflammatoires ou microcristallins, mais sa fréquence et les facteurs qui lui sont associés au cours de l’arthrose digitale (AD) sont mal connus. L’objectif de cette étude transversale est de déterminer la fréquence et les caractéristiques associées à l’arthrose radiographique des MCP chez les patients avec une AD symptomatique inclus dans la cohorte DIGICOD.
