Association of fractional excretion of sodium and urea and global enddiastolic volume index of patients with acute or chronic kidney injury

Eine frühzeitige Differenzierung der verschiedenen Ätiologien der Niereninsuffizienz ist therapeutisch relevant. In einer prospektiven Studie wurden die fraktionierten Exkretionen von Natrium (FES) und Harnstoff (FEU) von 36 Patienten zur Differenzierung untersucht. Zur Validierung diente der mittels transpulmonaler Thermodilution bestimmte Globale Enddiastolische Volumen Index (GEDVI). Erniedrige Werte von GEDVI und FES/ FEU sind nicht austauschbar. Änderungen von FEU und Serumkreatinin korrelieren signifikant. Jede akute Niereninsuffizienz wies eine erniedrigte FEU auf, jedoch war nur in 45% gleichzeitig der GEDVI erniedrigt. Alterations in FEU were significantly correlated with changes in serum creatinine. Every akute kidney injury showed decreased FEU, but only 45% showed concomitiant decreased GEDVI.
