MTM essentials for antiplatelet therapy in cardiovascular disease.

Oral antiplatelet agents have become an increasingly important treatment modality across the cardiovascular disease spectrum. The availability of newer agents with more potent antiplatelet activity, providing opportunities for improved efficacy at the cost of elevated bleeding risk, has increased the complexity of drug regimens. The pharmacist can play a prominent role in the care of patients who require and/or are receiving oral antiplatelet therapy. Studies have shown that incorporating pharmacists into primary care teams significantly increases the proportion of patients who receive guideline-directed antiplatelet therapy. Understanding the characteristics of the currently available oral antiplatelet agents will aid practicing pharmacists in the proper treatment of patients with cardiovascular disease.
