      We present the case of a 66 years old obese male patient with chronic hypertension, Crohn's disease and type 2 diabetes with non-compliance to treatment. He presents to the emergency department with a localized swelling in the perianal region of 10 days, which evolved with purulent secretion and fever. Broad-spectrum antibiotics were initiated as well as early debridation of necrotic tissue and support with total parenteral nutrition. The patient was discharged after 64 days of hospital stay with mesalamine 2 gr daily and azathioprine 50 mg daily. [ABSTRACT FROM AUTHOR]

      Se reporta el caso clínico de un paciente masculino de 66 años de edad, hipertenso, enfermedad de Crohn y diabetes tipo 2 con mal apego al tratamiento médico. Acudió a Urgencias debido a la aparición de una zona de induración en la región perianal de 10 días de evolución, posteriormente con salida de material purulento, y fiebre. Se le dio tratamiento con antibióticos de amplio espectro, debridación con resección del tejido necrótico y nutrición parenteral total. Ingresó al programa de curación, con anestesia, en cinco ocasiones para retirarle el material necrótico. Salió del hospital luego de 64 días con tratamiento de 1 g de mesalazina por vía oral cada 12 horas y 50 mg de azatioprina cada 24 horas por la misma vía. [ABSTRACT FROM AUTHOR]
