Pulpectomía con CTZ en paciente con anemia de células falciformes. Reporte de caso. (Spanish)

Pulpectomy with CTZ paste in a patient with sickle cell disease. Case report. (English)

The aim of this clinical report case was to perform non-instrumental pulp therapy with CTZ in temporary dental organs in a patient with sickle cell disease (SCD). Female 5 years 7 months old, diagnosed with SCD was treated at Regional Hospital of High Speciality of the Child "Dr. Rodolfo Nieto Padrón" in the pediatric dentistry area. At the clinical observation presented tendons and conjunctival palidity, at the intraoral level presented mucous pallidity, hypomineralization, caries in various dental organs (DO), the results of blood biometry were a hemoglobin: 6.04 g/dl, erythrocytes: 1.82 g/dl, leukocytes: 15 450 x 103/µL. Dental diagnosis was early childhood caries and pulpar necrosis in temporary molars 64 and 85 the first with dentoalveolar abscess. Non-instrumental pulp therapy treatment was carried out with CTZ in both temporary molars with pulp necrosis and as temporary glass ionomer restoration and subsequently steel crowns, temporary obturations in different DO for caries lesions. Result of CTZ treatment was effective, showing clinical and radiographic improvements. Non-instrumental pulp therapy with CTZ is a viable, fast and safe effective treatment in patients with SCD, prevents bacterial spread to periodontal tissues and minimizes systemic risk. El objetivo de este caso clínico fue realizar terapia pulpar no instrumentada con CTZ en órganos dentarios temporales en un paciente con anemia de células falciformes (ACF). Femenina de 5 años 7 meses, con diagnóstico de ACF, fue atendida en el Hospital Regional de Alta Especialidad del Niño "Dr. Rodolfo Nieto Padrón" en el área de odontopediatría. A la observación clínica presentó palidez de tegumentos y conjuntivas, a nivel intraoral presentó palidez de mucosas, hipomineralización, caries en múltiples órganos dentarios (OD), los resultados de biometría hemática fueron una hemoglobina: 6,04 g/dl, eritrocitos: 1,82 g/dl, leucocitos: 15 450 x 103/µL. El diagnóstico fue caries temprana de la infancia y necrosis pulpar en molares temporales 64 y 85, el primero con absceso dentoalveolar. Se realizó tratamiento de terapia pulpar no instrumentada con CTZ en ambos molares temporales con necrosis pulpar y como restauración temporal ionómero de vidrio y posteriormente coronas de acero cromo, obturaciones temporales en diferentes OD por lesiones cariosas. El resultado del tratamiento con CTZ fue efectivo, mostrando mejoras clínicas y radiográficas. La terapia pulpar no instrumentada con CTZ es un tratamiento efectivo viable, rápido y seguro en pacientes con ACF, evita la diseminación bacteriana a tejidos periodontales y minimiza el riesgo sistémico.
