Almost all patients presented with dyspnoea, yet only the minority of patients had clinical signs of congestion or fluid overload assessed by bioimpedance spectroscopy. B This article refers to 'Quantitative fluid overload in severe aortic stenosis refines cardiac damage and associates with worse outcomes' by K. Halavina I et al i ., published in this issue on pages 1808-1818. b The main symptom related to severe aortic stenosis is dyspnoea.[1] When aortic stenosis progresses to severe and a patient becomes symptomatic, aortic valve replacement is indicated. The discrepancy might point out that dyspnoea in patients with severe aortic stenosis is caused by other factors than fluid overload, such as decreased cardiac output and impaired organ perfusion.
