In general, institutional support presents itself as a potentially transformative managerial function, promoted by a dialogic action between a supporter and a work team, where all the subjects involved discuss the demands of the territories in a circle. For this, it is necessary that the action mentioned above is materialized by participatory and problematizing methodologies. Method: this is an experience report based on institutional support actions between two epidemiological surveillance services. These actions are part of an Intervention Plan (IP), whose purpose is to identify problem situations and propose interventions to transform reality and improve professional practice. General objective: to report the experience of institutional support actions promoted among workers and residents of the Epidemiological Surveillance Center of the University Hospital of Santa Maria - RS (NVEH) and COVID-19 Municipal Reference Center (CRM), which integrates municipal epidemiological surveillance. Results/Discussion: through the problem tree tool, possible causes and consequences for issues related to ineffective communication and poor information exchange were collectively elaborated. As a result, three main problems were identified: lack of communication between services (CRM and NVEH); difficulty in accessing information and incomplete COVID-19 diagnostic test reports. The problem tree, after formulated, discussed and rethought, helped in the analysis of the description above and was a tool that allowed consolidating the objectives of the meetings.
